Insurance Cases
All insurance cases require additional paperwork and records upkeep. To insure that I have the correct information on your specific needs and coverage please print out and complete a Patient Insurance Data form from the Forms area of the website. Send the completed form to me and I will contact you with your benefit information.
The following are some guidelines and general information regarding the types of insurance I work with.
On the Job Injury
If you have been injured at work or while performing your job a report should be filed with your employer as soon as possible after the accident. This report is filed with the Washington State Department of Labor and Industries (L&I).
L&I cases are handled by the L&I Dept. If you request massage as part of your treatment the Dept. will determine if massage is medically necessary. Most often you would be in the care of a M.D., Chiropractor, etc. who can prescribe massage for you. If approval is received from the Dept. of L&I, I will bill them directly and be in contact with them regarding the number of visits approved and other details.
Automobile Accident-With Personal Injury Protection Coverage (PIP)
If you have been involved and injured in any automobile accident and your auto insurance policy includes Personal Injury Protection Coverage (PIP) you are eligible for massage treatment to aid your recovery.
You will provide your insurance company information to me and I will bill them directly for your care. PIP coverage has a maximum amount allowed for all treatment pertaining to an accident case.
Automobile Accident-Lawsuit, Third Party Involvement, With no Personal Injury Protection Coverage (PIP)
If you have been injured in an auto accident that was not your fault, you were not cited for, are in litigation over and have no PIP coverage, you are covered for massage. This scenario is more complicated than others. It requires that a Lien is authorized by you and sent to your attorney so that I am guaranteed payment upon settlement of the case. These cases may take months or sometimes years to fully settle and therefore I am limited as to the number of cases I will accept.
Personal Health-Health Insurance Coverage
I am a provider for most health insurance carriers, such as Premara Blue Cross, Aetna, Guardian, etc. At this time Regence Blue Shield and Group Health are not available as they have been closed to any LMP applications for several years. If you are covered by Regence or Group Health I will try to provide you with a referral to another LMP so that you will receive the care you need.
